HCFA-1500 Envelopes

These envelopes are specially designed to work with HCFA-1500 Forms. Using these envelopes will save you time in filing and may expedite processing of your submitted claims. We have several different styles of HCFA-1500 envelopes available to fit your exact needs. All of our Right Window CMS-1500 envelopes work with both the 12/90 and 8/05 versions of the CMS-1500 forms. We also have special UB-04 Envelopes. All our claim envelopes are available plain or with your name and return address information printed on them. 

Your total cost is lower with us because our envelopes and our shipping together are less.

We also have a huge assortment of other business envelopes from basic white to deluxe letterhead stocks and dozens of single and double window sizes at very competitive pricing. We can include logos, colors and more and offer a wide array of paper stocks for the image conscious professional. We can even custom manufacturer envelopes to any mailable size if you have that special need.

These are Standard #10 size envelopes (9-1/2" x 4-1/8") with a window on the lower right side for the address on the folded HCFA-1500 form to show thru. This size is ideal for up to 8 claims per envelope. (If you usually file more per envelope see the size below). When you fold the form on the designated fold marks the address lines up correctly in the window. This saves the time of addressing an envelope for each claim you file. The quality construction of these envelopes ensures they arrive in a safe fashion. They are made with a deluxe flip & seal style self-seal closure with or without your return address printed.

We also have Gummed (wet seal) Claim Envelopes available for those that use automated inserters / sealers. The pricing is similar to our Self Seal Envelopes.


These are Jumbo size envelopes (9" x 12-1/2") for sending unfolded HCFA-1500 forms. By not folding the forms you reduce problems and delays when they are scanned at Medicare or the insurance company. (Forms that jam or don't scan correctly from being folded wrong must be manually handled and this causes delays in entry and payment). These Jumbo envelopes have a window on the upper right side positioned for the address on the unfolded HCFA-1500 form to show thru. This saves the time of addressing an envelope for each claim you file. They also feature a diamond shape border with the words "First Class" on the front and back so the post office can easily recognize this as First Class Mail and give it the expedited handling it is expected to receive. This is our second most popular claim envelope size and can hold up to 50 claims per envelope. They are ideal for attaching AOB, CMN, and multiple other documents to claims. The quality construction of these envelopes ensures they arrive in a safe fashion. They are available in the self-seal style with or without your return address printed.

We also have Jumbo Envelopes designed just for Resubmittals. They are similar to the above but have the word RESUBMITTAL in large Red letters printed on the front of the envelope.

Prices for printed envelopes drop a little as the quantity goes up. Please call for quotes on larger quantities. We have many other styles of envelopes available including envelope sets with preprinted Payment Return Envelopes already inserted inside #10 Statement Envelopes. Statement Envelopes come with and without windows and with Double Windows. Call us for all your envelope needs.

Shipping

Prices do not include shipping. We ship from 16 warehouses around the U S and will calculate the lowest shipping costs when you place your order. Shipping averages $8 - 12 / box of 500/1000 based on the weight and where you are in relation to our warehouses or printing plants. Most of the US is 1-2 days by ground via UPS/FedEx from one of our 16 shipping locations. Your total cost is lower with us because our forms are less and our shipping is less. Figuring the lowest shipping takes us more time but we save you dollars per box by only charging you for our actual discounted cost of shipping.
