HCFA-1500 Forms (CMS-1500)The HCFA-1500 (CMS-1500) forms illustrated here are the currently used Medicare claim forms and insurance claim forms. This is the primary medical office form used for billing physician services and they are also used by other medical arts professionals. (Hospitals use the UB-92 / UB-04 Form). These HCFA1500 (CMS1500) forms have the special Red ink for OCR scanning. People often call these Red HCFA1500 Forms but of course it is the ink that is Red not the paper. Whatever you call them is OK with us as long as you call us to order them. Of course we have a full line of HCFA-1500 Envelopes and we have other styles of HCFA-1500 (CMS-1500) Forms not illustrated here. 8/05 - NPI version now available The new version of the CMS-1500 form (usually called the 8/05 or NPI version) has many new fields for NPI numbers. This is really quite a bit different, not just one new field. There are more that 50 changes to the form. This was accepted by a few carriers beginning in October 2006, and by Medicare beginning in January 2007 and was scheduled to be the standard version beginning in April 2007. The old version of the form (with the date 12/90) was to be discontinued after March 31,2007 except in a few limited cases where some agencies had not gotten their systems working yet. (Mostly a few state Medicaid programs.) About the HCFA-1500 (CMS-1500) The HCFA-1500 was originally designed for Medicare claims but has since been adopted by most insurance companies. It is now known "officially" as the CMS-1500 since HCFA changed their name to CMS. But the form itself did not change at that time. The currently accepted form is known as the 8/05 version and has many new fields for NPI numbers. The data area and the data that goes there is the same on all the formats (laser, continuous, etc.) The HCFA-1500 was introduced in 1984 as a "regular" form printed with Black ink, but in 1990 this was changed to the special Red ink calibrated for OCR (Optical Character Recognition) scanning. Medicare and most insurance carriers scan these forms into their systems,. They only "see" the data you print on them not the stuff preprinted in Red. Even if you fill your forms out by hand (which means they will not be scanned for data) they still want the correct form. Data on hand written forms must be entered manually in the carriers computer systems which usually slows processing by a few days. Most carriers scan in all claims (even hand written forms) to keep an electronic image of the form so they can call it up an a computer screen if needed rather than dig through a file cabinet for it. Some large providers also archive images of their HCFA-1500 / CMS-1500 claims, but most keep a hard copy of each claim in the patient file for reference and re-submittal questions. This is also a common practice by those who file claims electronically. The special color of Red is a specific formulation of ink used to produce the specifically required color of Red used for optical scanning by Medicare and the Insurance Carriers. How light or dark the ink is does not matter a lot to the scanner. It is the exact color it is looking for. This color is not reproducible by most computer color printers which is why you can't print the forms as you need them. (Of course if you printed more than a few the cost of the color toners and paper would be more than we charge for the printed forms). Call (800) 828-1355 to order or with any question.
