No Fax? No problem! You can mail us this form and indicate to "Mail Invoice" under Special Instructions. 5) When you receive the Invoice, Mail your check to the address on the Invoice and reference Your Invoice Number on your check. 6) If it is a Money Order or Business Account Check your order will be processed the day we receive your payment.
